Forms for Students

At the beginning of your studies you need to sign the Statement, where you declare that you read and understood the examination regulations, and so you are allowed to take exams. Please, fill in the form and bring it to Mr. Potikha in CSiS Office (F.10.05):

Students who have been enrolled before the change of exam regulations (PO) in 2020 must apply for the change of the PO they are studying at. For that we kindly ask to fill in the form and bring it to Mr. Potikha in CSiS Office (F.10.05):

There are exams which can be taken only 3 times. They are called "restricted repeatable". CSim1, CSim3, and CFM1 are among them. The registration for this exam is needed via StudiLöwe. For several categories of students, as exchange or MATHS-DISC students, the registration and deregistration via StudiLöwe is not possible. In this case, you need to fill in the form and send it to the Central Exam Office to Mr. Philipp Depiereux (depiereux[at]uni-wuppertal.de):

In case you cannot attend an examination due to illness, it is obligatory to

  1. Inform your examiner as soon as possible before the time of the examination. Please send them an e-mail via your official university e-mail account. The e-mail should include the following information: your full name, matriculation number, study programme, name and number of the module/course as well as the date of the examination.

  2. Submit this form, as a PDF-file via the following link:

    Please, note that the certificate must be obtained immediately, generally no later than the day of the exam. Furthermore, the upload is only possible from within the university network. If you try to get the access outside the university´s network, A VPN connection may be necessary (Please, see: Information about VPN Access). The certificate you upload will automatically reach the responsible administrative staff.

The examination board of your study programme decides about granting or denying the certificate.
In case it is granted, the withdrawal will be recorded in StudiLöwe. In case the withdrawal is denied, you will receive a written statement.

Every 2 years the Wuppertal Foreigners Office (Ausländerbehörde) demands you to apply for the prolongation of your student´s visa, if you didn´t finish your studies. The form for Prolongation of Stay will be provided to the student physically by mail from the Wuppertal Migration Office. You need to fill it in and sent to CSiS Office (potikha[at]uni-wuppertal.de or csis[at]uni-wuppertal.de) together with the PDF-file of your actual Transcript of Records from StudiLöwe. If your academic achievements are poor, we will ask you to provide the plan on your future measures to finish the studies. When your documents will be signed and ready to collect, we will inform you about that per e-mail.
